1. This is the common presentation of the venous disease in over weight or obese individuals. a) minimal swelling around the ankle b) discolouration of the skin c) few prominent in the sub dermal and subcutaneous region. d)A small ulceration on the medial side of the ankle. e) disproportionate pain in the ankle region with or with out venous claudication.

2. There are many patients but they do not whom to contact for this problem management. This is managed at different levels by different specialist with different interests. The long term nature of disease which may progress steadily or intermittently certainly affects of the quality of life of rich and poor man person alike. The working men or women will be loosing work hours due to activation of the diseaes related symptoms intermittently. The costs of managing such symptoms and ulcers relating to chronic venous insufficiency is costing billions of dollars all across the world both in the west and east.
